> Full legal name as it appears on government documents (no nicknames, include all last names if you have multiple, etc.)

A valid US address that includes:

Street Address

Apartment number (if applicable)

City

State

Zip code

A valid Social Security Number (SSN)

A valid email address

A valid phone number
